Before the review
Confirm the current medicine list against at least two sources, including what the patient actually takes.
During
Check indication for every item, doses against kidney and liver function, and interactions including over-the-counter and herbal products.
After
Document what you changed and why, and tell the next person in the chain. A good note saves the following shift an hour.
